This is a 6 hr session with options for additional practice session. Each participant receives follow up coaching from our team to prepare for their first learning review and to debrief its completion.

Action items from incident reviews can pile up or languish on a To Do list. We will look at the systemic barriers and facilitators to prioritizing and implementing safety and resilience improvement efforts when teams are already overloaded and collaboration across teams is low.
